Straps at a vertical when on wrist?

I don't think the strap is vertical because your wrists are small, it's because its not seasoned yet.
If you're worries about the size, get the Mark XV instead, it's 38mm in diameter, new or used doesn't matter. You can always get the strap replaced at JW watches, I had mine on a Kevlar looking strap before I sold it.
(Btw, WOS at Centrepoint has a brand new Mark XV)
The IWC Mark XVI is 39mm in diameter, hardly any difference but it's numerals are closer towards the edge of the dial perimeter, making it look larger than the Mark XV.
I'd say don't go for the steel bracelet as the IWC Pilot watches look best with leather straps, otherwise, the BP will come with bracelet version already.
You can even find the straps with rivets to resemble the Big Pilot! I guess that'll make it the small pilot!

haha thanks ocean for the poison. think i go try one more time. If really vertical and I do mean vertical as the lugs seem to be protruding from the end of my wrists, then bo bian not buying leI dont need another watch, I dont need another watch, I dont need another watch, I dont need another watch.........
